CARDIOVASCULAR CLINIC REVIEWS

Invites are issued once a year to all patients diagnosed with one or more of the following conditions:-

Diabetes

Heart Disease

Chronic Kidney Disease

Stroke

Hypertension

Peripheral Arterial Disease.

If you have more than one of the above conditions, you will only be called in once and your combination of conditions will be reviewed at the same time.

It is important to attend your annual review even when you are feeling well so we can monitor your condition, reduce your risk of complications and to live as full and healthy a life as possible.

The first appointment will be with our phlebotomists for blood, blood pressure and urine tests.  Please bring a mid-stream urine sample with you (white topped bottles can be obtained from the surgery)

The second appointment, which will be at least 1 week later, will be with a Nurse in our Virtual Clinic, using NearMe video consultation or telephone, where your initial results will be discussed and you will be asked questions about your general health and given advice on healthy living.
